Care Communities is the only volunteer-based nonprofit that provides free, team-driven practical assistance for people confronted with life-threatening illness who need simplified and compassionate care in an era of decreasing personal connection. Each client (‘Care Partner”) has a team of 3-4 volunteers who provide emotional support and practical assistance so they don’t have to confront their illness alone. Volunteers provide companionship, and help with day-to-day activities such as cooking, pet care, light housekeeping, laundry, yard work, errands, grocery shopping, and transportation to medical appointments. Our Care Managers provide ongoing case management services to our Care Partners.
In 2016, 300 volunteers gave 17,000 hours in working with 145 Care Partners and their family members. The majority of our Care Partners live alone or have little to no support system. 76% of our Care Partners are age 55 years or older, with 36% and 65 and above. All Care Partners live at or below 200% of the Federal Poverty Level. In 2016, 60% of our Care Partners had a cancer diagnosis and 40% had a diagnosis of HIV/AIDS.
